Filling in a form

Task 1: Work in pairs. Answer the following questions.

(Làm việc từng đôi. Trả lời các câu hỏi sau.)

1. On what occasions do you have to fill in a form?

2. What sort of information do you often have to provide when you fill in a form?

Trả lời:

1. I have to fill in a form when I enroll a class or a course, book a hotel room or apply for a job.

2. When I fill in a form, I have lo provide my personal information or my background.

Task 2: Forms do not usually ask questions, but they ask for information. Match a line in A with a question in B.
